Vision Insurance
Vision insurance contributes a certain dollar amount towards the routine eye exam fees and/or contact lens fitting fees. Certain vision insurance plans provide an allowance or discount for hardware (glasses or contact lenses).
Every vision insurance has different stipulations, so to be able to maximize your benefit it is important for you to understand your particular plan. If you need details about your vision insurance you can contact the number on the back of your insurance card or contact your HR department.
Vision insurance is NOT applicable towards LASIK/PRK evaluations, OrthoK evaluations/fittings, low-dose Atropine therapy, or IPL therapy. However, an HSA or FSA can be used as a form of payment.

Medical Insurance
Medical insurance is used for eye related medical conditions. At times, during your routine eye exam, there may be medical conditions that require further investigation or tests to help us thoroughly care for your eyes. Having your medical card information in your file is valuable to us. This is different from billing your vision insurance for a routine eye exam and is subject to any deductibles, copays, or co-insurances.

Out of Pocket Policy
Insurance coverage is a strict contract between you, your employer, and your insurance company. We do not dictate your insurance coverage. You are responsible for any copays or out of pocket costs for services rendered and all payment is due in full at the date of service. If you are unsure about your insurance plan details, please contact your insurance company or HR department before making an appointment. All appointments made without insurance details or out of network insurance are subject to our out of pocket prices.
